Becoming a tissue donor is a simple process that will change the lives of those in need of transplants.

Most important:
Discuss your wish to donate with your next-of-kin. Their knowledge of your desire to be a donor helps them to carry out your wish.

If you are 18 or older, just fill out a Uniform Donor Card.

Fill in your name, indicate whether you wish to donate all organs and tissues when you die or list specific organs and tissues, and sign and date the card.

Then sign your name in the presence of two witnesses. Your witnesses should also sign the card. Preferably, witnesses will be next of kin.

Carrying this card with you at all times will alert medical professionals and inform them of your wishes. But the only guarantee that your wishes will be conveyed to the appropriate eye, tissue, or organ bank is for your next-of-kin to be aware of your desire to be a donor. Discuss donation with those closest to you.
